Kommune og region går sammen om sundhedsforbedringer

Borgerne skal opleve sammenhæng på deres vej mellem sygehus, egen læge og kommune. Det er ambitionen med en række konkrete initiativer, som Odense Kommune og Region Syddanmark lægger op til i den ny sundhedsaftale for 2011-2014.
